Information Required for the Installation or WaterOne provides the design, cost estimate, and installation of all new water mains for development. All water mains must be installed by WaterOne or its contractor. Anyone requiring a water main extension or upgrade must enter into a Main Extension and Petition Agreement with WaterOne prior to installation. PLEASE PROVIDE THE FOLLOWING TO TERRY SCHULZE, NEW MAIN EXTENSION ADMINISTRATOR: 1. Name, mailing address and telephone numbers (including facsimile, mobile, pager, etc.) of the property owner. If the property owner is not an individual, include the type of legal entity, name and title of authorized agent, and contact information.2. If the Petitioner is someone other than the property owner, provide similar information on the Petitioner as indicated above for the property owner.3. Name of general contractor for your project, project superintendent and phone numbers (including construction trailer, facsimile, mobile, pager, etc.).4. The maximum total fire flow required by the appropriate fire jurisdiction.5. Site geotechnical information6. A digital file of the plat as recorded with the Register of Deeds or certificate of survey with legal description conforming to the Digital Drawing Specifications for subdivision plats submitted within Johnson County.7. Both paper copies and digital files of the site development plan as approved by the appropriate municipality. Digital files may be submitted through the WaterOne ftp site in AutoCAD format.dwt. For information on accessing the FTP site, contact Mallee Morris, Administrative Secretary, at 913-895-5741. Include the following:· Overall site plan and proposed utility plan. Include all future phases of development and the anticipated sequence of construction.· Street, sanitary sewer and storm sewer plans, including plan-profiles and cross-sections.· Grading plan, including any berms. · Landscaping plans, including any entrance markers or monuments. · Erosion and sediment control plans. · All existing utilities. Minimum SUE level D (Refer to Construction Institute/American Society of Civil Engineers publication 38-02, Standard Guidelines for the Collection and Depiction of Existing Subsurface Utility Data).NOTE: IF ALL EXISTING UTILITIES ARE NOT SHOWN, THE EXCAVATION PERMIT PROCESS COULD BE DELAYED BY THE MUNICIPALITY HAVING JURISDICTION AND AFFECT CONSTRUCTION SCHEDULING. 8. For commercial or multi-family developments, include the desired location of thefollowing: · The domestic water service. · The fire line, if required. · The fire department connection(s), if required.
